Case management is a service which provides assistance to persons with developmental disabilities and their families and/or guardians in accessing needed services, supports, and funding. Responsibilities of case managers include:
• Assessment of needed supports and services
• Support planning
• Coordination of services
• Advocacy
• Assisting with life transitions and portability of service
